The Harris Center for Mental Health and IDD (The Harris Center) is on the hunt for a dedicated Service Coordinator who is passionate about empowering individuals with intellectual disabilities. In this vital role, youll help our clients navigate various servicesfrom medical and psychiatric assistance to residential and financial support, and beyond. Your mission is to unlock the full potential of person?directed planning processes, shaping meaningful outcomes according to each individuals unique needs and aspirations. Youll serve as a critical bridge between consumers, legally authorized representatives, program providers, and other key collaborators.

Your Role In Action

• Conduct comprehensive assessments, adhering strictly to our departments policy and procedural guidelines.
• Ensure client welfare through regular, at least quarterly, monitoring to each consumers unique needs.
• Maintain the highest standard of data integrity by updating consumer files as necessary.
• Be responsible for timely documentation, ensuring all records are updated within our established timelines.
• Uphold a safe and healthy environment by complying with all safety and health requirements and staying updated with mandatory training programs.

What Qualifications You Will Need

Education

• Bachelors degree (B.A.) from a four?year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
• OR any Bachelors degree with one (1) year experience working with persons with intellectual and developmental disabilities.

Experience

• Minimum of one year experience in the human services field preferred.
• Experience working with persons with intellectual disabilities preferred.
• Must possess effective written, verbal communication, and organizational skills.
• Must be able to work autonomously in a fast?paced environment.
• Effectively communicates and works with consumers, family members, visitors and fellow employees in a manner that demonstrates knowledge of cultural diversities.

Other Key Qualifications

• Must maintain a valid Texas Drivers License and be insurable under Agency policy.
• Must have reliable transportation.
