Location: Conroe TX
Schedule: Full-Time | Monday-Friday 8am-5pm
Pay Rate : 18$/HR

LT Home Healthcare is seeking a detail-oriented and dependable Medical Records Clerk to join our Compliance Division. This vital role helps maintain the integrity of patient documentation and ensures compliance with HIPAA, Medicare/Medicaid, and state healthcare regulations. Key Responsibilities

  • Maintain and organize medical records for skilled and unskilled home care services in accordance with internal protocols and legal standards.
  • Audit electronic and paper records for accuracy, completeness, and regulatory compliance.
  • Track submission timelines for physician orders, plans of care, and treatment authorizations.
  • Collaborate with nursing and administrative teams to correct or complete documentation.
  • Ensure all medical records meet Medicaid, Medicare, and private payer documentation standards.
  • Respond to medical records requests securely and in compliance with HIPAA.
  • Support external audits and surveys by preparing and organizing necessary documentation.
  • Report any non-compliance or irregularities to the Compliance Officer.
  • Uphold confidentiality and security of all patient health information.
  • Participate in ongoing compliance training and support policy development.
Qualifications
  • High school diploma or GED required; Associate's degree or Health Information Management certification preferred.
  • At least 2 years of experience in a medical records or compliance-related role (home healthcare experience a plus).
  • Familiar with HIPAA, CMS regulations, and Texas Medicaid documentation requirements.
  • Proficient with electronic medical records systems (AlayaCare, Medius, or similar).
  • Strong attention to detail and organizational skills.
  • Ability to work independently and meet strict deadlines.
  • Excellent communication and interpersonal abilities.
  • Discreet and professional when handling sensitive patient information.
Work Environment & Physical Requirements
  • Office-based role within the Compliance Department.
  • Minimal travel; may occasionally lift up to 25 lbs (e.g., file boxes).
  • Prolonged periods of computer and desk work.
