Position Summary:
The Client Care Manager (Residential Service Director) is responsible for ensuring high-quality care and compliance for individuals served by developing and managing program plans, coordinating services, and maintaining accurate documentation. This role ensures that all client needs are met in alignment with regulatory standards and company policies, while fostering strong communication among care teams, families, and external partners.

Essential Duties and Responsibilities:

  • Responsible for development, revision, and implementation of individualized program plans; ensures documentation is current and health/safety needs are met effectively and in a timely manner.
  • Manages all aspects of client care, including case management, compliance with regulations, admissions, financial reviews, service reporting, and metrics tracking.
  • Assists in crisis situations to prevent escalation and ensure the well-being of individuals served.
  • Ensures individuals are protected from abuse, neglect, exploitation, and financial impropriety by adhering to reporting protocols and company policies.
  • Oversees timely incident reporting and documentation; follows up to ensure resolution and compliance with all regulatory agency requirements.
  • Reports all census changes promptly; ensures proper procedures are followed for client admissions, discharges, and transfers, maintaining accurate records at all times.
  • Maintains consistent communication with case managers, state coordinators, guardians, and medical professionals. Conducts and documents monthly contact with families/guardians.
  • Manages service utilization per authorized contracts, ensuring efficient and appropriate use of approved hours; advocates for changes when goals are not being met.
  • Ensures all client documentation is stored appropriately and complies with HIPAA, state, federal, and internal standards.
  • Runs 30, 60, and 90-day renewal reports; completes all scheduling and documentation to prevent lapses in service authorizations. Coordinates with billing and inputs data into Sandata or other company systems.
  • Participates in regulatory surveys and audits; responds to deficiencies and contributes to the development of corrective action plans.
  • Conducts ongoing chart reviews using standard audit tools to ensure survey readiness and regulatory compliance.
  • Collaborates with Nursing, Behavioral, and Dietary teams to maintain accurate and up-to-date clinical information.
  • Provides regular updates to client records, ensuring all medical documentation is current and correct.
  • Attends all required client-based and behavioral meetings; submits written reports and documentation as needed and provides follow-up as required.
  • Other duties as assigned.
Qualifications
  • Bachelor's degree in human services, social work or other related field or will accept equivalent, verifiable, successful work experience preferred.
  • Preferred a minimum of 1 year of management experience in a multi-site environment.
  • Prefer exposure to developmental disabilities environment.
  • Valid Texas Driver's license, good driving record, and reliable transportation.
Knowledge and Skills
  • Requires extensive mental activities including the ability to: use educated and intuitive judgement, advise, counsel, influence, debate, negotiate, organize, plan and synthesize concepts.
  • Ability to work in a demanding environment.
  • Must be able to make wise decisions under stressful circumstances.
  • Must be proficient in Microsoft Office including Word and Excel. Equipment skills required: using a PC, calculator, copier, fax machine, scanner.
